Make an Appointment

You can book appointments with any of the clinical staff 4-6 weeks in advance. You can do this over the phone or via our online booking system.

If you don’t know how to access the online system, one of our reception staff will be happy to help you register.

Urgent Appointments

We release a limited number of urgent appointments each morning. These are for people who have urgent medical needs, NOT for routine health queries.

Please only use this form for routine appointments. If you feel you require an urgent appointment, call us on 01654 702 224 or use one of our other access points.

All patient information is considered to be confidential and we comply fully with the Data Protection Act. All employees have access to this information in relation to their role and have signed a confidentiality agreement.

Information may be shared, in confidence with other NHS organisations in the interests of patient care.

If you have any concerns regarding patient information, please ask to speak with the Patient Support Officer.

Telephone Calls

We offer both Telephone and face to face appointments for Urgent on the day appointments and routine appointment. Please note that urgent on the day appointments will receive a call at first instance

Home Visits

We always prefer to see you in the surgery where we have much better facilities for examining and treating you. Home visits are for patients whose illness or disability means they cannot get to the surgery.

Please phone reception before 10am if you need a home visit. A clinician will usually call you back to decide whether you need telephone advice, a nurse or doctor visit or hospital attendance.

Referral Queries

If you have not heard from the hospital within 6 weeks please contact the hospital/department directly. Please be aware that waiting times on referrals are based on hospital availability and is out of our control

Chaperone

You are always welcome to bring someone with you to an appointment. If you prefer we can offer a member of staff to act as a chaperone during your consultation.
